Thank you for your inquiry regarding updating the address for your ELT states with DDI Technology. Below you will find the required state documents & instructions for each state program. To get started, complete and return your documents per the instructions below. Please be sure to download each of the documents. All documents are fillable once downloaded. .DDI’s Contract’s Team will be in touch to notify you that your address change has been updated and will also advise if a new ELT number has been issued. You will also receive a case number. You can track the progress of your address changes for each state by logging into the DDI Help Page to view your cases. ** Please follow all instructions/requirements. Each state requires different documentation for an address change. If you have any questions, please reach out to our Contract’s Team.

Applications and procedures to update a Lienholder Address in each ELT State: Please be sure to download each of the documents. Arizona Complete the following: • Arizona ELT Application : From the Action Requested section, select “Change of Financial Institution Address.” • Arizona Lienholder Information Update Form • Typed or Handwritten applications are accepted. You will receive a new ELT number for an address change in California. Please see the CA implementation calendar below: IMPORTANT! Applications must be typed. 2/10/2026 There are 5 sets of formsthat need to be fully completed to enroll in the CA ELT program. Once these forms are processed by CA, you will be implemented into the CA ELT program. • Along with your REG 671, you must also submit “Business Documents”. “Business Documents means a Lienholder’s license or charter by state or federal banking authoritiesto loan money forthe purchase of a vehicle and proof of authority to conduct auto loan businessin California. See CA Proof of Authority Instructions and Approvalsfor Business Documents CA ELT - Proof of Authority Instructions- with attachments • All forms must be physically signed – except for the Reg 677. The Reg 677 does allow electronic signatures. • The state will not accept forms over 60 days old. CA Cover Sheet - (Check the box “Address Change” and complete the form.) -Please submit as a single PDF. CA 671 - Page 1 – Please complete form, please place inked signature in Section 4. Please leave Section 5 blank for DDI to complete. Page 2 – Please place inked initials in Section 7, also complete Section 8 and sign with inked signature. Please scan completed 671 & Business Documents together as 1 attachment. CA 673 Please complete the formwith physicalsignature. CA 677 - **REG Form 677 – Please have every user complete, sign, and return who may work with CA ELT/Titles, etc. Please scan all 677 formstogether and submit 0as 1 PDF.
